PB2 E627K and HA S227N in a H5N1 Human Isolate in Turkey

But they might also favour human-to-human transmission. This is because the polymerase change helps the virus to survive in the cooler nasal regions of the respiratory tract, and the haemoagglutinin mutation encourages the virus to target receptors in the nose and throat, rather than lower down in the lungs. The virus is thought to be more likely to spread through droplets coughed from the nose and throat than from infections lower down.

The above comments confirm that the presence of HA S227N (also called S223N)and PB2 E627K in the same isolate in Turkey is cause for concern.  The PB E627K had been almost exclusively found in mammalian isolates including all human isolates of serotype H1, H2, H3.  It appeared in bird isolates from Qinghai Lake, 

New H5N1 PB2 sequences have been made available,  They are listed below and include 2005 isolates from wild birds from Jiangxi as well as domestic poultry isolates from Indonesia, Vietnam, and several provinces in China.  However, the only sequence containing E627K is the environmental sample from Qinghai Lake.  These data indicate E627K is a recent acquisition for H5N1and became fixed in wild bird populations during and after the Qinghai Lake outbreak.

It’s appearance in a human isolate in Turkey, along with HA S227N, is cause for concerm
